Course Description

Provides in-depth examination of case management as a model of service delivery within the human services field. Students will study the phases and essential components of case management, including intake, assessment, service planning, implementation, monitoring, and evaluation. Emphasis is placed on the role of case management in coordinating services, promoting client self-sufficiency, and ensuring continuity of care across diverse systems and populations. The course integrates theoretical foundations with applied practice, incorporating experiential learning activities to develop core competencies such as interviewing, goal setting, documentation, advocacy, and interagency collaboration.
